This is required for me to communicate and meet with your child(ren) during remote learning under certain circumstances. This would be required to speak with your child/children either individually or in a group regarding personal or private matters and concerns; it is not necessary for PALS or Lunch Bunches.

Please click here to download and fill out the Counseling Therapy Release and Consent form. This allows me to communicate with your child(ren)'s physicians and therapists.
