Registration Documents Information

The documents needed for registration are collected via our online Registration System.  Please provide the following:

1. Proof of Residency
The suggested residency documents are:
  • One primary proof of residency, such as a deed, mortgage statement, lease, lease renewal letter, property tax bill, driver's license for the address, contract of sale or other similar proof of property ownership, tenancy or residency.
  • One secondary proof of residency, such as a utility bill, cable/phone bill, financial statement, insurance statement or contract, employment documents and pay stubs, voter registration or other government mailings, or any similar document.
  • If you reside with a host family, and have no documents in your name for that address, please submit a Homeowner's Affidavit and submit proofs of residency in the homeowner's name.
  • If you are unsure about what residency documents to submit, please submit those that you have, and the school district will make a determination based on the documents you submitted to us.
2. Proof of Student Age and Custody
  • Please provide us with a copy of the student's birth certificate 
  • If there are custody agreements that change custody from what is indicated on the birth certificate, please include those.
  • Foreign birth certificates do not need to be translated
3. Parent Photo ID
  • Please provide us with a photo ID with your name, such as a driver's license, passport, work ID or similar item.
4.  Medical Clearance to Attend School
  • Please submit a copy of the student's immunization record.
  • Please submit a current health physical for the student (less than 12 months old).  Here is a sample form
  • You can register without medical records, but the student will not be allowed to attend without medical clearance from the nurse.  You should only skip providing immunization records if you are confident that you will be able to provide them to the school nurse prior to the start of school.
  • If you have copies of other important medical records our nurse should have, such as a food allergy emergency care plan, please include them.
5. Other Optional Items
  • If the student has an IEP, please submit a copy of the current IEP if one is available
  • If you have the student's latest report cards or a copy of their transcript, please include it.
